Level of Care
Oak Wood Estates provides 3 levels of care based on the individual needs of each resident. The daily
rate is based on the level of care needed. Level 1 is the base level and very limited assistance. Level 2
is moderately increased assistance on reminders to perform activities of daily living. Level 3 requires
frequent assistance and support with activities of daily living including cueing or increased assistance
with health needs.
Oak Wood Estates Level 1 (Base Rate) is provided to all residents. This rate includes:
- 3 daily meals planned by a licensed dietician
- A variety of snacks available 24 hours daily
- Utilities
- Housekeeping services
- Planned activities and educational events
- Parking for those with vehicles
- Emergency call system (monitored 24 hours per day)
- Safety checks (staff on premises 24 hours per day)
- Basic cable television
- Daily trash pick-up
- Room and grounds maintenance
- Community and family participation in activities or social events
(Residents are responsible for telephone service if they elect this service.)
Level One:
Minimal Care: Occasional supervision, assistance or reminders to perform some daily activities but is
independent in other activities. Will be charged for ancillaries.
Level Two:
Moderate Care: Requires more substantial support with some activities of daily living, while needing
only minimal assistance with others. Will be charged for ancillaries.
Level Three:
Heavy Care: Needs frequent and comprehensive assistance with activities of daily living. Staff will
assist with dressing, assist with incontinence care and one person assisted transfers, example, from bed
to wheelchair. Will be charged ancillaries.
Note: Level of Care will be determined from an assessment scoring tool. If a resident requires more
than level three care, the provider will do a significant change assessment. Will meet with resident and
family to discuss if Supportive Living is appropriate for the resident.
